Service Agreement Terms and Conditions
US Medical Licensing, LLC IS NOT affiliated with any medical board.
I am hiring you (US Medical Licensing, LLC) to assist me in applying for state medical licensure, registrations, and credentials as specified in our Service Agreement, as selected during the online signup process or as requested from time to time either verbally or in writing by myself,
my employer or representatives.

I understand the information I provide you will be transcribed on online or paper applications or provided by my employer or representatives and I affirm its accuracy and completeness. I understand the scope of the

Services will be limited to:
preparation of any license applications and supporting documentation; processing and followup of related verification requests; and management of the application process through completion of same. I understand I remain responsible for fees payable to the licensing boards and others to whom an application is submitted for registration or credential.

I acknowledge:
You cannot guarantee compliance with licensure application deadlines; you cannot and do not guarantee approval of my state medical licensure application; and you are not responsible for changes in eligibility requirements by any licensing board.

Excessive Malpractice or Employment History
Typically, your Services include only application preparation, credentials verification requests, and follow-up and management of the application process until the license or credential is granted. If a client has more than 3 malpractice cases or more than 10 employment/staff privileges that must be verified, we may bill extra depending on how much time is required to collect the verifications. This will be determined on a case by case basis, and will be discussed with the client before any verifications are requested. Please keep in mind that we will spend whatever amount of time is needed to complete the licensure process and additional billing is extremely rare.

International Credentials
US Medical licensing, LLC will assist foreign medical graduates with obtaining licensure, however; we do not have the ability to collect international credentials (international medical-school transcripts). When assistance is required by our client to obtain international credential verification, we outsource this step of the process to a board approved credential verification service.

Holds
I agree that if my file becomes dormant (no activity for 60 days or more), you will place a hold on my file and I may be required to pay you a reactivation fee.

Document Reproduction
US Medical Licensing, LLC will provide unlimited alteration to any of the documents that it prepares.

The client will only be billed $50 per hour in the following situations: Excessive ‘personal preference’ alterations (examples include but are not limited to: font, spacing, or pitch changes), or reproduction of documents that were lost, damaged, rendered outdated because I failed to submit it to you timely or because I allowed my file to become dormant.

Omission of Information
I agree that if I omit information required for you to perform the Services, I may be required to pay you additional fees to prepare application amendments or subsequent documentation requests. Additionally, I acknowledge that any omission of pertinent information, whether intentional or otherwise, may be reflected on documents you prepare and subsequently result in adverse actions (fine, discipline, etc.) from medical boards or other entities and I accept full responsibility of any such outcomes.

Client Caused Delays
An action by the client that keeps us from performing at our normal speed of operation may cause the client to be billed for additional time. Some examples of client caused delays may include withholding information, not responding to correspondence within a reasonable amount of time (2 weeks), holding your completed application for longer than a month before submitting it to the board.

Licensing Time Frames
US Medical Licensing, does not guarantee licensure by a specific date. All time frames listed on our website are estimations of time frames based on our experience only and are not a guarantee. We ask that our clients do not enter into any contracts or obligations for work until they have received their full license.

Online Payment Agreement
By submitting your payment via our online system or PayPal you are acknowledging that you have read and agreed to US Medical Licensing, LLC polices (privacy policy, refund policy, licensing procedures and fees) outlined on our website.

Additional Licensing Fees
The licensing fees mentioned on US Medical Licensing website are our fees only and do not include state board fees, shipping/handling and merchant fee charges or third-party fees. These fees are in addition to the fees listed on our Services page. Shipping fees are assessed per item invoiced.

Refund Policy
If the licensure process is canceled for any reason before your license is issued you may be entitled to a partial refund per application as follows:

Work Completed Refund
Phase 1 (Research) Up to 75% of service fee
Phase 2 (Application preparation) Up to 50% of service fee
Phase 3 (Credential verifications) Up to 15% of service fee
Phase 4 (Follow-up) No refund available

US Medical Licensing, LLC Reserves the right to refuse service or to cancel service at any point during the licensure process due to unprofessional behavior exhibited by a client. If Healthcare Licensing Services cancels service, the client will be entitled to a prorated refund according to the parameters listed above.

Identifying Information
In general, you may visit the site without telling us who you are or providing any information about yourself. Generally, US Medical Licensing requests identifying information when you:

2. Place and order.
3. Request information or files from technical support or customer service.

In these instances, US Medical Licensing will ask for your name, address, e-mail address, phone number, and other necessary information needed to provide you with these services.

What Medical Licensure Group will do with your information
If you choose to provide our agency personal information for any of the purposes outline above, this information is retained by US Medical Licensing, LLC and will only be used by US Medical Licensing, LLC for the purpose intended and to support your customer relationship with our organization. We will not add you to a mailing list or newsletter.

What others may do with your information
US Medical Licensing, LLC does not share, rent, or sell any personally identifying information to any outside organization for use in its marketing or solicitations. Occasionally, US Medical Licensing, LLC may use agents who will have access to some of your personal information to perform services for US Medical Licensing, LLC (such as verifying credentials or document translation). However, they are required by law to keep the information confidential and may not use it for any purpose other than to carry out the services for US Medical Licensing, LLC. In
addition, US Medical Licensing, LLC may also share aggregate information about its customers and its web site visitors. None of this information will contain any personal, identifying information about our users.

Third party links
US Medical Licensing, LLC does provide links to other internet sites for information purposes. Other internet sites have separate privacy and data collection practices.
